I to suffer from this issue when I play resulting in many bowled/LBW out.

I believe it happens with every batsman. If you get your eyes in, you will see the ball onto the bat. But if you are still unsettled and not seeing the ball, you tend to feel for the ball and poke at it, happens due to lack of confidence, lack of ability and/or initial nerves.
Have a look at any innings by any batsman, chances are the batsman will initially play the ball away from the body[push at it] and as he settles in, crosses the 20 mark he will see the ball onto the bat and play the ball closer to the body.
